The Change Appearance feature in the game allows you to change nearly everything that you can change in the initial character creation process. When you see your own reflection in the mirror, click on the “Change Appearance” option. If you see Johnny, exit the mirror function and start again. It’s a gameplay feature related to the story of the game. Note that sometimes you may see Johnny Silverhand reflected in the mirror. Interact with the mirror to access the additional features. By the way, this works with all bathroom mirrors in any of the apartments you rent later on. When in your apartment, stand in front of the mirror in the bathroom. Simply complete the main quests until you are given your own apartment in Night City. To be able to change V’s appearance and body features in Cyberpunk 2077 in-game, past the original Character Creation process, you need to have unlocked V’s apartment.
